1.  Do I need an Identification Card?

No, Midwestern Dental members are not required to show an ID card to receive services. All members are immediately identified in the computer system as to eligibility. Simply identify yourself as a Midwestern Dental Plan member and your provider will proceed with services. You will, however receive a New Member Packet to welcome you.

2.  Can I go to my private dentist with Midwestern Dental Plans?
No, Midwestern Dental is a staff model managed care system. All services must be rendered by a Midwestern Dental Center provider. Midwestern Dental offers you seven convenient locations to receive services.

3.  Can I transfer to another dental provider if for some reason I need to?

Yes, in most cases you are able to transfer to another Midwestern Dental provider, however it is subject to approval from Midwestern Dental Plans.

No, all members must attend the same dental center.

One of two ways, you may call Midwestern Dental Plans to select a provider after you enroll or at the time you fill out your application define your selection.

After your effective date, simply call the dental provider you selected at the open enrollment and schedule your appointment.

7.  What happens when I have an emergency?

You can call the dental center you have selected, explain your emergency and they will inform you how to proceed. If in the event the dental center is closed all Midwestern Dental Centers have a twenty-four hour emergency phone service. The patient should call the center normally attended and listen to the recorded massage for the center hot-line instructions.

8.  Who are your dentists?

Upon request, Midwestern Dental Plans will forward a doctors list to you with the names, practice specialty, dental school attended, year graduated and the location they participate with.

9.  Do I see the same dentist each time?

Managing your family’s dental health is a priority with Midwestern Dental Plans. At your initial visit you will see a Dental Director for a review of your dental health and recommended treatment. Follow up care is provided by the available treatment dentist. Specialty care is referred to a participating Midwestern Dental specialist. (Oral Surgery, Periodontics and Orthodontics)
