No, Midwestern Dental members are not required to show an ID card to receive services. All members are immediately identified in the computer system as to eligibility. Simply identify yourself as a Midwestern Dental Plan member and your provider will proceed with services. You will, however receive a New Member Packet to welcome you.

One of two ways, you may call Midwestern Dental Plans to select a provider after you enroll or at the time you fill out your application define your selection.
After your effective date, simply call the dental provider you selected at the open enrollment and schedule your appointment.
You can call the dental center you have selected, explain your emergency and they will inform you how to proceed. Managing your family’s dental health is a priority with Midwestern Dental Plans. At your initial visit you will see a Dental Director for a review of your dental health and recommended treatment.
